Certificate of Insurance

Certificates of insurance may be required by contract in several areas throughout campus. Contracts for the rental of an event venue, lease of a vehicle or equipment, or the award of a research grant all contain insurance requirements. These contractual terms may be met by the issuance of a certificate.

Certificate of Insurance Request

Certificates of insurance are issued and maintained by Insurance and Financial Risk. To receive a certificate, a request form must be completed. A  contract or agreement must be attached to your request.  A draft contract or agreement is acceptable for certificate of insurance purposes.  Please allow 5 business days for processing.
